Remove a subscription item

Take an item off its subscription. It is marked removed, deleted: true, rather than deleted. Only the source that owns its subscription may remove it.

Its Summerbase id, or an identity ref: a source slug and that source’s own id for it, joined by a colon — stripe:si_123.
